Death Notification

To inform us of the death of a policyholder, please call Customer Service at 1-800-362-0700, or send a written notification to the address above.  Please include a copy of the Death Certificate.

In the event of a policyholder's death, a refund may be owed for unearned premiums. Unearned premium refunds will be made payable to the estate of the deceased policyholder. To request that the refund be issued to a trust or an individual, please complete the appropriate affidavit below, and return it by mail or fax to 610-965-6962.
